Q. How do I make a claim?
A: Claim Forms are available on line. You can download PDF versions of these forms by selecting "Claim Forms" in the Customer Service menu opposite. Once the form is printed, both you and your doctor should complete, sign and post it to us at the address shown.

Q. I have lost my insurance policy. How do I get a duplicate?

Q. How can I pay for my premium if I have missed my local agent?
